Effective March 1st, 2023: The FMNP Graduate Database is no longer active on our website. 

Registration Graduate ID - If you are an FMNP graduate looking for your FMNP graduate ID for registration purposes, this is no longer necessary. We have removed this requirement from the registration form and no longer issue FMNP graduate IDs.

FMNP graduate verification of FMNP courses completed – If you wish to confirm a list of FMNP courses that you have completed (e.g., if you have lost your completion certificate), please send an email to fmnpcoordinator@gmail.com with your first and last name, and the county/counties where you took your class(es) if you have that information. 

3^rd party request for verification of FMNP courses completed by an individual – If you need to verify FMNP courses completed by an individual as an employer, potential employer, or to award CEUs or in-service credits, please send an email to fmnpcoordinator@gmail.com with the name of the individual and your contact information and organization. We will provide a list of FMNP courses completed by the individual specified. We will not release any personal data or contact information for any individual.
